Use this on-line form to request a REDcap project

Please note that this form is for Medical Sciences Division users only. All external users need to request access via their MSD REDCap supervisor.

If you are not an Medical Sciences Division (MSD) registered user, please contact your MSD REDCap supervisor, who will arrange access on your behalf. You will be treated as an external user to MSD.

If you are an MSD IT user but do not currently have an active account, please apply for an MSD IT account first. Otherwise, this form will be rejected.

This information is only required if you are requesting a new project. This is where the cost of your new REDCap project will be charged to.

New Project Name

If you enter a project name here we will create a NEW project for you which will be subject to the registration fee outlined in the REDCap Service Description. We make a small administrative charge per project created in REDcap to cover our administrative work in maintaining and configuring the system for your use as described in the service description.

It is important that you read the MSD REDCap service description before registering interest in MSD REDCap Service.

Agreement
