The call invites applications to support real-world integration of preventive technologies into the workflow of community care settings, such as Neighbourhood Health Hubs, Community Diagnostic Centres, community pharmacies or outreach/mobile services.
This funding opportunity provides the critical bridge for proven technologies in a community/primary care setting (TRL5) to move beyond standalone pilots and become embedded, interoperable components in community care.
