Request additional access or changes to Departmental or Unit shared drives or network resources
Please download this form for requesting additional access to Departmental or Unit shared drives or network resources, for making changes to any existing access rights, for removing access rights if a user leaves the department or Unit, or for a user name change.
After filling the form, you need to have it signed by the owner of the data to be accessed, nominated authoriser, your Departmental Administrator or their nominated representative (University Departments only).
Forward the completed form to your local MSD IT office. You can scan and email the form to firstname.lastname@example.org
We will contact you by email to inform you when your job has been completed and access granted.
